Charles Bray Forman III, 65, of Southside died March 6, 2018, with his family by his side. Chaplain Jay Beggs will officiate. Burial will be in Crestwood Cemetery. Crestwood Funeral Home  announcing.

He's survived by 4 daughters Melissa Forman Amedee of Georgia, Riley Forman Bates and

Mindy Alicia Bowen ,of Southside and April Marie Forman of Dothan.

Son in Laws Ruben Amedee , John W. Bates and Daniel W. Bowen

Grandchildren,  Ruben Amedee Jr., Collin Amedee, Hailey Jade Bates and Layla Aubrey Bowen.

He is the son of the late Martha Doris Forman.

He loved fishing with his family, flying his drones, and collecting movies and coins.

Pallbearers are John W. Bates, Alexander J. Palmer, Daniel W. Bowen, Michael W Loggins, Jason A Bates and Ruben W. Amadee.
Matt Rose will be officiating.
